This website allows you to request transcripts for courses you completed through the Teacher Enhancement Program at Colorado School of Mines.
Transcripts for courses taken by matriculated students at Colorado School of Mines are not available through this website. If you are seeking such transcripts, please visit the Colorado School of Mines Registrar's Office at https://mines.edu/registrar/transcripts/.
Neither the Teacher Enhancement Program nor Colorado School of Mines can be responsible for meeting individual deadlines for credits toward teacher licensure and/or salary increases. Official transcripts for current course work can be printed upon request approximately two weeks after the instructor submits course grades to the Teacher Enhancement Office. Individual letters of completion, unofficial transcripts, and online viewing of your student record are not available. (Only matriculated Mines students who pay student fees can have online access.) Please note our new address, which is available here or at the bottom of this page along with other contact information, if you are mailing a transcript request form that you received during a class or that you obtained through this website.
Costs and Payment
The cost for transcripts is $10.00 USD per copy requested. Payment can be made online through this website or by mail. Online payment is accepted only by credit or debit card. The following cards are accepted:

How to Order
To order your transcript(s), please enter your personal information in the appropriate spaces that appear below. You can choose to pay online using a credit/debit card or by mail with a check or money order. Upon successful completion of an online payment, or if you choose to pay by mail, you will be provided a Request for Official Transcript of Record form. A backup copy of the form will also be sent to your email address. You should print and complete the form and return it — with your payment if paying by mail — to the Teacher Enhancement Program office. Your transcript(s) will be sent to you by mail to the address(es) you provided.
